Passmore <br />TELEPHONE NUMBER - <br />Hillsborough - 732 -8181 <br />Chapel Hill - 967 -9251 <br />Mebane - 227 --2031 <br />Durham - 688 -7331 <br />Extension 2000 <br />PURPOSE: <br />To request approval to add Medical Personnel Pool and <br />Triangle Home Health Care, Inc. to the group of In -Home care <br />providers used to provide service as funded through contract <br />with Triangle J Council of Governments, at a rate not to <br />exceed $9.50 per service hour. <br />BACKGROUND: <br />Triangle J Council of Governments will provide a total of <br />$23,043 in federal and state funds for In -Home care. The <br />match of $2,560 has already been budgeted by the Department <br />on Aging. <br />The County Departments of Aging and Social Services have <br />identified a need for short -term relief for caregivers of <br />clients who do not have funds or have limited amounts to pay <br />for assistance. These are family situations where the older <br />member cannot be left alone due to a mental or physical <br />condition. Additionally, a need has been identified for the <br />performance of critical tasks in order to maintain self <br />sufficiency within the home - such as shopping, preparing <br />meals and cleaning. In order to spend all of the funds <br />available and maintain a quality provider pool, several <br />providers will be utilized. The In- Home.care program will be <br />administered by the Department on Aging but coordinated by <br />the interdepartmental Case Management Team (Aging and Social <br />Services). <br />RECOMMENDATION(S): <br />Manager recommends approval and Chair to sign provider <br />agreements. <br />1 <br />
