10 <br />I <br />ORANGE COUNTY C. A. T. DRIVER AGREEMENT <br />This agreement entered into this 1st day of July, 1991 <br />by and between the County of Orange on behalf of the <br />Department on Aging, hereinafter referred to as the "County" <br />and Orange - Person- Chatham Mental Health Center, Inc. on <br />behalf of Children's Learning Center, hereinafter referred to <br />as "OPCMHC ". <br />The purpose of this agreement is to outline the <br />conditions under which the County and OPCMHC will <br />cooperatively provide human service transportation under <br />C. A. T. (Coordinated Agency Transportation) system. <br />SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY <br />I. To provide a certified driver for Children's Learning <br />Center Southern Orange route Monday through Friday for daily <br />pick -up and take -home of clients between the hours of 6:30 - <br />8:30 a.m. and 2:30 - 4:30 p.m. <br />2. To provide a back -up driver in case the regular driver <br />is not available. The Transportation Supervisor will notify <br />the Children's Learning Center Director of regular drivers <br />absence. In the event the Transportation Supervisor is <br />unable to place a short term driver on a daily basis, the <br />Director will be notified in advance to place a staffperson <br />for a period not to exceed two consecutive working days. <br />3. To employ a C. A. T. Transportation Supervisor to handle <br />any problems and to assure there is a driver available. <br />4. The staff of the C. A. T. Transportation Program agrees <br />to comply with Division of Mental Health, Mental Retardation <br />and Substance Abuse Services minimum standards for area <br />programs and their contract agencies Section L. 1300 - <br />Transportation Services. <br />SPECIFIC RESPONSIBILITIES OF OPC MENTAL HEALTH CENTER <br />1. To reimburse Orange County at the rate of $8.00 per <br />hours for a three and one -half hour minimum each day of <br />operation within 30 days after billing. <br />2. In the event OPCMHC requires training beyond driver's <br />current level, it shall be OPCMHC's responsibility to: <br />a) dictate the level of training; and to <br />b) provide.the prescribed training without cost to <br />the County. <br />
