ORANGE COUNTY C.A.T. PROGRAM <br />VEHICLE LEASE AGREEMENT <br />FN: CATOPC- CT.DOC <br />This agreement entered into this 1st day of July, 1991 <br />by and between the COUNTY OF ORANGE hereinafter referred to as <br />the COUNTY and ORANGE PERSON CHATHAM MENTAL HEALTH CENTER hereinafter <br />referred to as the Mental Health Center. <br />The purpose of this agreement is to outline the conditions under <br />which the COUNTY OF ORANGE and The Mental Health Center will <br />cooperatively provide human service transportation to Orange County's <br />elderly, infirmed, and disadvantaged as a participating member of the <br />C.A.T. (Coordinated Agency Transportation) system. <br />SPECIFIC RESPONSIBILITIES OF THE COUNTY OF ORANGE <br />I. To lease the following identified vehicles or similar size <br />at the hours specified: <br />1989 Dodge Van (14 pass)- CAT #19, 7 -9am, 6:30- 4:30pm <br />1990 Dodge Van (15 pass)- CAT# 7, 8 -10am, 3:00- 5:OOpm <br />2. To provide all insurance, fuel, maintenance repairs and <br />replacement for each leased vehicle. <br />SPECIFIC RESPONSIBILITIES OF ORANGE PERSON CHATHAM MENTAL HEALTH <br />1. To reimburse Orange County at the per mile rate of $.35 for a <br />van due 30 days after billing. <br />2. To operate and schedule the use of the leased vehicle(s) on be- <br />half of human service clients and approved groups, according to <br />the Orange County Transportation Board policy and procedures. <br />3. To park all leased vehicles at the CAT parking lot (300 West <br />Tryon St., Hillsborough) when not in use except CAT# 19 to be <br />parked at the Efland Cheek Elementary School in Efland. <br />4. To complete all required vehicle operation reports and submit <br />to the Orange County C.A.T. Supervisor in the Department on <br />Aging. <br />5. To establish criteria for client rider eligibility. The criteria <br />is as follows: (1) Availability of seats, (2) Inability of <br />client to provide own transportation, (3) Where client lives and <br />his ability to get to pickup point, and (4) Behavior on vehicle. <br />EMERGENCY PROCEDURES. The agreed upon emergency procedures will be as <br />follows: Two -way radios will be installed in each vehicle with the base <br />located at the Department on Aging. In the event of a mechanical <br />emergency, the driver will radio to base for help. In case of a medical <br />emergency, the driver will first radio to 911 for aid and then to the <br />base. Base operator will be responsible for contacting The Mental Health <br />Center. At no time will the driver leave riders unattended. <br />The term of this contract shall be from July 1, 1991 through <br />June 30, 1992, unless sooner terminated by either party giving 30 days <br />prior written notice to the other party of intention to terminate. <br />IN WITNESS WHEREOF, COUNTY OF ORANGE and ORANGE PERSON CHATHAM <br />MENTAL HEALTH CENTER have executed the Agreement of the date first aba� <br />written. <br />WA <br />COUNTY OF ORANGE AGENCY <br />Chair or Designated Official <br />By <br />Date Date <br />Chair or Designated Official <br />
