Between The University of North Carolina <br />School of Dentistry for its <br />Department of Dental Ecology <br />and OCHD and NCMH <br />Page 2 of 5 <br />NOW, THEREFORE, in consideration of the premises and of the following <br />mutual promises, covenants, and conditions, The School of Dentistry, The Hospital <br />and The Health Department agree as follows: <br />1. The Health Department will reimburse The School of Dentistry at the <br />close of each quarter for 1/4 the total salary ($24,000) and fringe benefits, <br />p'orker's Compensation ($72.00), Unemployment Insurance ($72.00); FICA <br />($1,836.00), Hospital Insurance ($2,657.00), Parking ($210.00), Interns Permit <br />($50.00), Total Fringe ($4,897.00) of one postgraduate year one resident <br />beginning July 1, 1991 (quarterly totals - salary $6,000.00 fringe $1,224.25 <br />grand total quarterly reimbursement $7,224.25). <br />2. The School of Dentistry will provide the services of one postgraduate <br />year one resident to The Health Department for the purpose of rendering <br />comprehensive dental services of 4 1/2 days /wk. <br />3. School of Dentistry will bill The Health Department at the close of <br />each quarter for the salary /fringe of one postgraduate year one resident. <br />4. The person whose services are to be provided pursuant to this Agreement <br />is, for all purposes, an employee of The School of Dentistry. <br />5. While the North Carolina Memorial Hospital Oral Medicine House Staff <br />are on rotation at the Orange County Health Department, their professional <br />liability insurance coverage will be provided by the Self- Insurance Program for <br />Professional Liability of The North Carolina Memorial Hospital and University of <br />North Carolina School of Medicine with coverage of at least $1 million. <br />6. This Agreement shall run for a period of one year, from the 1st day of <br />July, 1991 to the 30th day of June, 1992, and shall be renewable thereafter. <br />6 <br />T. <br />i._ <br />
