3 <br />4. The contracting agency has read, understands, and <br />agrees to follow the revised (July 1, 1991) Division <br />of Aging standards for all applicable services (ie. <br />Senior Center Operations, Institutional Respite <br />Care, In -Home Aide Services, Information and Case <br />Assistance, Care Management, Home Delivered Meals, <br />Congregate Meals) and cost sharing policies and <br />procedures. Said standards and cost sharing <br />policies and procedures are hereby incorporated into <br />the FY 91/92 contract renewal response. Contractor <br />agrees to utilize the Service Cost Sharing form <br />(DOA -402) for applicable services. <br />5. A new or revised Description of Services for each <br />service listed in Para. 4 above shall be <br />incorporated as Attachment D. <br />6. All provisions of and attachments to the FY 90/91 <br />bid specifications and contract, except Schedule 1- <br />Scope of Services, Attachment A- Catering <br />Provisions, and Appendix C -lb Approved Menu Format, <br />carry forward except where they conflict with the <br />newly incorporated DOA standards or provisions <br />stated herein. In such case, the newly incorporated <br />DOA standards will prevail as will provisions stated <br />as part of this FY 92 Contract for Aging Services. <br />A new Schedule 1, reflecting any changes pursuant to <br />Para. 4(b) Terms of Renewal, is made a part hereof. <br />A revised Catering Provisions (Attachment A) is <br />hereby incorporated as part of this contract, still <br />subject to all provisions of Para. 37. A new <br />Appendix C -1b is made a part hereof. All services <br />required herein shall be completed on or before June <br />30, 1992. <br />7. A new Certificate of Insurance pursuant to Para. 30 <br />Indemnity and evidence of insurance pursuant to <br />Para. 31 Insurance, shall be furnished to COG by <br />July 31, 1991. <br />8. The Unit Cost Worksheet (Exhibit 7(e)) which <br />reflects actual cost and revenue data for fiscal <br />year FY 92 shall be submitted by July 31, 1992. <br />9. Services relevant to this agreement to be provided <br />by Contractor, will proceed continuously from FY 91 <br />into FY 92. Any services altered as a result of the <br />revision of DOA standards effective July 1, 1991, <br />will commence July 1, 1991 with applicable <br />specifications from FY 91 to carry forth. <br />10.
