NOW THEREFORE, in consideration of the premises, and the mutual covenants and agreements hereinafter contained, the parties hereto agree as follows: 1. All provisions and attachments of FY 91 Contract for Aging Services will apply except as stated herein. 2. COG agrees to contract for Case Management Services in the revised service forms of Information & Case Assistance and Care Management. Payment will be made on the basis of the accepted FY 92 line item budget. 3. COG agrees to contract for Senior Center Operations and Neighborhood Advisors. A revised Senior Center Technical Questionnaire shall be incorporated as Attachment B. A revised Description of Service for Neighborhood Advisors shall be incorporated as Attachment C.
