A <br />E <br />AGREEMENT BETWEEN <br />The University of North Carolina at Chapel Hill <br />for its School of Dentistry <br />AND <br />The Orange County Health Department <br />THIS AGREEMENT, made and entered into this first day of February, 1991 <br />and between the University of North Carolina at Chapel Hill, hereinafter by <br />to as "The University" for its School of Dentistry; and the Oran referred <br />Orange <br />Department, hereinafter referred to as "The Health De art g County. Health <br />p ment ". <br />WITNESSETH: <br />WHEREAS, The University proposes a six -month clinical trial of a toothpaste <br />developed for the prevention or reduction 'of gingivitis or plaque in human <br />the University > and <br />y proposes to conduct a portion of the clinical trial at The Health <br />Department in Hillsborough, NC and <br />WHEREAS, the Health Department desires to contract with the University <br />be reimbursed for costs involved (i.e. clinic space, supplies conducting to <br />the <br />clinical trial. <br />NOW, THEREFORE, in consideration of the premises, The University <br />Health Department agree as follows: and The <br />1- The Health Department agrees to provide to the <br />University the use of clinical chairs in The Health <br />Department in Hillsborough, on Mondays, Tuesdays, and <br />Fridays on reserved days (approximately 20 days) in the <br />dental clinic. <br />2. The University shall pay The Health Department $300 per full day and <br />$150 per half -day for the use of the facility. If instruments are <br />used that belong to The Health Department, an additional $25 will be <br />added. Payment shall be made on a quarterly basis. <br />
