By June, 1992, 1 fitness facility (local spa) will provide membership <br />discount for new members from the employee population. <br />8. By June, 1992, develop and implement a tracking system to verify referral <br />outcomes of 75% of Orange County government employees referred for high <br />blood pressure and cholesterol. <br />9. By June, 1992, 2 staff members participating in health promotion activities <br />will receive continuing education in a health promotion.or related training <br />program. <br />D. QUALITY ASSURANCE <br />A Quality Assurance Plan for the Personal Health Services Division is on <br />file. A Quality Assurance Committee for the health department has been <br />established and meets at least quarterly for quality assurance activities. <br />Program and record review by Adult Health Section Supervisor will occur at <br />quarterly or six month intervals. All quality assurance findings and <br />corrective action will be documented in written reports. Information. <br />collected will include, but not limited to, client assessment and test <br />results; record adults; worksite promotion, implementation, participation and <br />barrier data; participant evaluations; outcome analysis for meeting program <br />objectives for local six month and annual reports and state performance <br />report. A policy and procedure manual includes all services provided by the <br />program and is reviewed and updated yearly. <br />Health Professionals involved in the Health Promotion Program: <br />Jerry Ann Gregory RN, BSN, Health Promotion Program Coordinator, with 22 <br />years of public health nursing experience. <br />Belinda Jones, MPH, Health Education Section Supervisor, with 10 years <br />experience in public health education, <br />Ona M. Pickens, RNC, BSN, Adult Health Section Supervisor, with 12 years <br />experience in public health nursing. <br />Laurel Bond, RD., MS., Nutrition Program Coordinator, with 12 years or <br />experience providing nutrition services. <br />Eileen Kugler, MSN.
