Target Population <br />Page 2 of 4 <br />F, 91 -92 <br />92 -550 68 <br />Contract Number <br />Adult Health <br />Activity <br />Death from cardiovascular disease is higher among non - whites than whites <br />in Orange County indicating the need to target black residents in the <br />county. Employment statistics for North Carolina as a whole show that <br />black persons are more likely to suffer unemployment, chronic illness and <br />premature death. Education level is generally high in Orange County with <br />78% completing high school, 13% of blacks attend college compared to 47% <br />for whites. Northern Orange County is more rural. The population tends <br />to be older, less affluent, have less access to health care resources and <br />less awareness of health care issues. <br />Selected activities to reach the targeted population will include inhouse <br />clinics and community outreach services. lnhouse clinics include a walk -in <br />adult screening clinic (i.e. BP screening) two days a week and a more indepth <br />appointment clinic for health risk identification, counseling, referral and <br />follow -up service one day per month (serve 168 individuals with 30% being <br />non - whites). lnhouse clinics are open to Orange County residents. Health <br />educators and the Health Promotion Coordinator will provide information about <br />services to black community groups and churches to increase utilization by <br />target group. <br />Community outreach services include group education and individual screening, <br />education, counseling, referral and follow -up for identified risk factors for <br />cardiovascular disease. Selected community screening sites will target black <br />community and church groups (five to ten sites serving 100-200 persons) <br />primarily in Northern Orange County. <br />4. community Organization Involvement <br />Organizations involved in health department initiatives will include the <br />Department on Aging, American Heart Association and the Agricultural <br />Extension Agency. <br />B.
