Health, and Natural Resources <br />Division of General Services <br />Children and Youth <br />Office, Section, or Branch <br />e County Health Department <br />Contractor <br />CONTRACT ADDENDUM <br />School Health Program <br />Page 3 of 4 <br />Fy 1992 <br />X35/ <br />92-542t-068 <br />Conrract Number <br />Child Health <br />Activity <br />The Health Department will have a written agreement* with the local school <br />district(s) it serves, which reflects joint planning and includes the <br />following: <br />1) program goals and objectives <br />2) roles and responsibilities defined for each agency including the <br />provision for nursing supervision <br />3) a description of the process for developing written policies /procedures <br />4) provision for annual review /revision of the agreement <br />*A copy of the current agreement, signed annually by both agencies, shall be <br />submitted to the MCC regional nursing consultant by October 1, each year.
