(New Applications Only) — Describe how potential volunteers, or persons to be served, or their <br />representatives, were involved in the development of this application. <br />2. a. (New Applications Only)— Specify your plan for establishing the project's Advisory Council. <br />Membership should reflect the population characteristics of the project service area and <br />include volunteers, persons served or their surrogates, representatives from ethnic and <br />minority communities, the handicapped, and community leaders. <br />b. (Renewal Applications Only)— Attach a list of current members of the Advisory Council <br />showing their organizational or community affiliation. <br />See attached. (Page - 13. ) <br />3. a.
