INFORMATION CONTACT:T PAM - <br />JONES <br />FORM OF LEASE OPTION Telephone Number - <br />EXT 2650 <br />Hillsborough --732 -8181 <br />Chapel Hill - 967 -9251 <br />Mebane - 227 -2031 <br />Durham -688 -7331 <br />PURPOSE: To consider exercising a two year option on the lease for <br />the Health Clinic space at Carr Mill Mall, Carrboro, NC. <br />BACKGROUND: In March, 1979 Orange County leased 4136 square feet for <br />Medical Clinics and offices at Carr Mill Mall. The cost <br />was $26,884 per year. <br />In September, 1979, the County received grant funds from <br />the Robert Wood Johnson Foundation to operate a primary <br />care dental clinic. An option to lease an additional 1444 <br />square feet of space was exercised in order to provide <br />space for the clinic. The cost for this space was $9,386 <br />per year, bringing the total annual lease amount to $36,270 <br />or $6.50 per square foot. <br />The lease amount is adjusted annually by the "Consumer <br />Price Index for Atlanta, Georgia, . . . for the first month <br />of the first lease year and for the corresponding month in <br />each lease year thereafter" (November, in our case). <br />For your information, a schedule of renewal amounts is <br />provided: <br />Inception of <br />lease <br />Expiration of <br />lease <br />Annual lease <br />amount <br />3 -13 -79 11 -1 -82 $36,270 <br />11 -1 -82 10 -31 -85 44,640 <br />11 -1 -85 10 -31 -88 54,405 <br />11 -1 -88 10 -31 -90 60,281 <br />
