Resolution "Seeking permission to Amend FY 1990-91/FY 1991 -92 Application for <br />Section 18 Funding" attached as Appendix A. <br />II. Project Supporting Documentation <br />A. Project Description <br />L Describe current and any proposed new or restructured service to be provided <br />under the project labeled as Appendix B. Include in the description services for <br />the general public and special efforts to meet the needs of the elderly and <br />handicapped. <br />2. Complete as Appendix C "Vehicle Inventory Chart ". <br />3. Provide a copy of current general public fare policy as Appendix D if changed <br />since the 1990 -91 Section 18 Application submittal. ` <br />B. Project Coordination <br />Complete Appendix E "Human Service Agency Coordination ". <br />C. Public Involvement <br />Complete copies of the following labeled as Appendix F: (1) copy of the <br />Public hearing notice affadavit of publication; (2) minutes of the public <br />hearing; and (3) a description of how the applicant addressed comments received. <br />D. Private Participation <br />1. Verify Appendix G "Surface Public Transportation Providers." <br />2. Provide a description of private participation activities as Appendix H that have <br />occured since the filing of the FY 1990 -91 Section 18 application. <br />"Note: When submitti ng the Section 18 Application, applicants are requested to provide a document <br />with original signatures and 'one copy. <br />
