NEW POSITION REQUEST FORM <br />Department: Health Division: Personal Health /Child Health <br />Proposed Classification Proposed Grade /Step: 66 -3C <br />Title: Social Worker II Proposed Hiring Date: December, 1990 <br />Number of Positions Number of Hours Per Week: 20 <br />Requested: 1 <br />Permanent: X =Note: This half -time position will <br />extend the existing approved <br />Social Worker II /Baby Love <br />Coordinator position to full <br />time. <br />Primary Duties and Minimum - Education and Experience: <br />Note: 20 hours -per week will be spent in Baby Love Coordinator duties. <br />This portion of the position is on the Maternal Health Program budget. <br />Duties: <br />Primary Responsibility <br />Provide Child Service Coordination (CSC) Services for eligible children and <br />their families. This includes: <br />(1) Provide community outreach to promote the program. <br />(2) Facilitate identification and enrollment. <br />(3) Provide Child /Family assessment, to include parent /child <br />relationships, physical needs, developmental status, educational <br />needs, social. /emotional factors, understanding /expectations of <br />family, available support systems, financial needs, transportation <br />needs. <br />(4) Develop Service Coordination plans. <br />(5) Arrange and coordinate services. <br />(6) Follow up and monitor service plan. <br />(7) Reassess and evaluate service plans. <br />Resource to Staff <br />Meet regularly with Supervisor and MCH team to exchange information about <br />CSC activities. Provide monthly activity report. Assist PHN's wiht <br />patients who have psychosocial needs. Keep abreast of the services of <br />community agencies in order to provide effective referrals. Communicate <br />update information to MCH staff. <br />Continuing Education <br />Attend inservice educatio programs and workshops that are pertinent to <br />social worker intervention and CSC. Read current publications to update <br />knowledge. <br />Education and Experience: B.S.W. from accredited school of social work, <br />M.S.W. preferred. One year of experience in health and human services <br />working with pregnant women and their families. <br />0 <br />
