CHILD SERVICE COORDINATION <br />AGREEMENT TO SERVE AS COORDINATIVE AGENCY <br />We have reviewed the Manual for Child Service Coordination and agree to be <br />the coordinative agency for this activity in our county. <br />this responsibility includes: We understand that <br />I. Encouraging the development of a service coordination system in th <br />county by discussing the service with all interested a n <br />eciese <br />including interagency councils when available; g <br />2. Developing a memorandum of understanding with each agency that <br />agrees to participate, clarifying interagency responsibilities, <br />eligibility, sharing of information and referral procedures; <br />3. Acting as the actual provider of service coordination to eligible <br />children from birth to age 5 with special needs, unless or until <br />another agency is deemed more appropriate through a process <br />outlined in the Manual. The provision of service coordination is <br />based on family needs, and not based on the availability of a <br />reimbursement resource, <br />4. Entering into HSIS identification information and periodic status <br />information submitted, by participating agencies within the county. <br />5. Maintaining 'a log of families receiving service coordination, and <br />their current source of the service. <br />6.
