V <br />STATE OF NORTH CAROLINA <br />COUNTY OF ORANGE <br />Page 11 <br />CONTRACT RENEWAL <br />THIS CONTRACT RENEWAL, made and entered into this the <br />1st day of July 1990, by and between The orange County <br />Health Department, party of the first part, and The <br />University of North Carolina at Chapel Hill for its <br />Department of Pediatrics in the School of Medicine, party of <br />the second part. <br />WITNESSETH: <br />THAT WHEREAS, a Contract has previously existed between <br />the same parties which are parties to this agreement, the <br />subject matter of said existing contract being the provision <br />of services of a nurse practitioner and a pediatrician in <br />the Hillsborough Health Department Clinic and it being dated <br />July 1, 1983, and amended July 1, 1984, July 1, 1985, July <br />1, 1986, July 1, 1987, July 1, 1988, and July 1, 1989. <br />WHEREAS, a copy of said previously existing Contract is <br />attached to this document and is hereby incorporated by <br />reference as though fully set forth; and <br />WHEREAS, the parties to this agreement desire to <br />continue and renew the relationship set forth in the said <br />previously existing Contract; <br />NOW THEREFORE, in consideration of the mutual promises <br />and agreements and any sums to be paid, the parties agree <br />that the previously existing Contract which has been <br />incorporated by reference herein is hereby renewed and <br />reaffirmed on identical terms and conditions except as set <br />forth below: <br />1. The term of the Contract will begin on the 1st day <br />of July, 1990, and end on the 30th day of June, 1991. <br />2. Paragraph #1 of the previously existing Contract <br />dated July 1983, is amended to read as follows: <br />A pediatrician from The University will provide <br />pediatric services in the Health Department's <br />Hillsborough Clinic one half day each week (4 hours) <br />during the contract period. On most weeks this <br />pediatrician will be Dr. Maria Portilla, a Clinical <br />Assistant Professor in the Department of Pediatrics. <br />The half day will be devoted to an adolescent clinic. <br />A fully trained pediatrician will be provided from the <br />staff of the Division of Community Pediatrics on those <br />occasions when Dr. Portilla is unable to attend. <br />During the 3 months from July through September, Dr. <br />
