Dr. Jonathan Klein will sign as supervising physician for the nurse <br />practitioners. All other members of the Medical Group will sign as <br />backup supervising physicians. <br />D. The Medical Group will provide professional liability coverage for <br />its members. The Department will provide professional liability <br />coverage for the nurse practitioners and other employees. <br />E. The Department will pay the Medical Group at the rate of two <br />hundred and ten dollars ($210.00) per week on the following <br />schedule: <br />1. For the months of November and December, the Department will <br />pay an advance amount of one thousand, six hundred and eighty <br />dollars ($1,680.00) by the 30th of November, 1990. <br />2. For -the months of January, February, and March, the Department <br />will pay an amount of two thousand, seven hundred thirty <br />dollars ($2,730.00) by the 31st of January, 1991. <br />3. For the months of April, May, and June, the Department will pay <br />an amount of two thousand, seven hundred and thirty dollars <br />($2,730.00) by the 30th of April, 1991. <br />F. This agreement shall run for a period of eight (8) months from the <br />1st day of November, 1990, to the 30th day of June, 1991, and shall <br />be renewed from year to year thereafter upon written notice <br />executed by both parties. <br />G. This agreement or its renewals may be terminated at any time, <br />without penalty by either party, provided that written notice of <br />such termination is furnished to the other party at least sixty <br />(60) dayt prior to termination. in the event of such termination <br />any payment made shall be prorated to the date of termination. <br />H. This agreement contains the entire understanding of the parties and <br />shall not be altered, amended or modified, except by an agreement <br />in writing executed by the duly authorized officials of both <br />parties. <br />
