STATE OF NORTH CAROLINA <br />COUNTY OF ORANGE <br />AGREEMENT BETWEEN <br />ORANGE COUNTY HEALTH DEPARTMENT <br />/Tf7 <br />ORANGE FAMILY MEDICAL GROUP, P.A. <br />Page 2 <br />a <br />THIS AGREEMENT made and entered into this the 1st day of November, <br />1990, by and between the Orange County Health Department, hereinafter <br />referred to as "The Department," and the Orange Family Medical Group, P.A., <br />hereinafter referred to as the "Medical Group." <br />W I T N E S S E T H: <br />WHEREAS, the Department desires to contract with the Medical Group for <br />family medical professional services to improve the care of patients in the <br />the Department's Child Health clinic; and <br />WHEREAS, the Medical Group desires to provide these services for the <br />Department; <br />NOW, THEREFORE, in consideration of the premises and the following <br />mutual promises, covenants, and conditions, the Department and the Medical <br />Group agree as follows: <br />A. A physician member of the Medical Group, primarily, Jonathan Klein, <br />M.D., will attend the Child Health clinic in the Hillsborough <br />facility every Wednesday morning from 8:15 a.m. to 11:45 a.m. In <br />addition, a physician from the medical group will be available to <br />back -up phone coverage of the Child Health clinic every Monday <br />morning from 8:00 a.m. to 12:00 p.m. <br />B. The duties performed by the Medical Group include: <br />1. Approval of clinic protocols. <br />2. Primary supervision of the nurse practitioners who are <br />employees of the Department. <br />3. Co- signing charts of patients seen by the nurse practitioners <br />of the Department. <br />4. Medical consultation services to Child Health patients on <br />Wednesday mornings. <br />5. Well child medical services on an occasional basis when the <br />nurse practitioner is not available. <br />6. Periodic continuing education consultation for nurse <br />practitioners and other Child Health staff. <br />
