r <br />STATE',OF NORTH CAROLINA <br />COUNTY OF ORANGE <br />AGREEMENT BETWEEN <br />WAKE COUNTY OPPORTUNITIES, INC. <br />mJA <br />THE ORANGE COUNTY HEALTH DEPARTMENT <br />Page 2 <br />THIS AGREEMENT is made and entered into this the fifth day of November, 1990 by <br />and ,between wake County Opportunities,, Inc. of Raleigh, N.C., for its Headstart <br />- Program referred to as "Headstart "; and the Orange County Health Department of <br />Hillsborough, N.C., for its Dental Clinic hereinafter referred to as "Dental <br />Clinic." <br />WITNESS,ETH: <br />WHEREAS, Headstart desires the services of the Dental Clinic to provide <br />dental services to the children who are enrolled at the Cedar Grove Center; <br />WHEREAS, The Dental Clinic wishes to provide dental services for Headstart; <br />NOW, THEREFORE, in consideration of the premises and the following mutual <br />promises, covenants, and conditions, Headstart and the Dental Clinic agree as <br />follows: <br />1. The Dental Clinic will provide dental examination, assessment, <br />treatment, and educational services for approximately sixty (60) children, 3 -5 <br />years old, enrolled at the Cedar Grove Center. <br />2. The Dental Clinic will assure that its employees and contract officers <br />are covered with professional liability insurance while rendering services to the <br />Headstart children. <br />
