Service Priorities <br />Which three services represent the most important service <br />__needs for-your-county. that - -shou l d _ba- funded ..in the -State <br />Plan for serving older Adults? For a listing of <br />potential services to consider, you may wish to refer to <br />the "Service Adequacy Assessment" form or the <br />"Supplemental Resource Survey." However, you do not need <br />to restrict your priority services to those on these <br />lists. Feel free to add other services, if appropriate. <br />Please list specific services rather than general service <br />categories and please do not combine issues. Example of <br />preferred format: "We need more trained, paid personnel <br />to provide personal care to adults in their homes." <br />* We need more respite care funds and manpower) both in- <br />home and community based to relieve caregivers of frail <br />elderly. <br />* We need more Chore service (.funds and manpower) to <br />maintain the home environment for frail elderly. <br />* We need additional adult day care service lboth social <br />and day health care), especially in central /northern <br />Orange County. <br />Other honorable mentioned services are more home health <br />care, case management, medical transportation, home <br />repairs and affordable housing assistance. <br />S. Issue Identification <br />In addition to the service priorities identified above, <br />there are important issues relating to the development, <br />implementation, or use of services for older adults which <br />should be addressed in the State Plan for Serving Older <br />Adults. Please use the back of this page to list all such <br />issues that you feel are relevant. real free to include <br />additional pages if needed. Again, try to 'make your <br />comments as specific as possible.
