S2 <br />b. Any failure of a pumping station, seNver line, or treatment facility resulting in a <br />by -pass directly to receiving waters without treatment of all or any portion of the <br />influent to such station or facility. <br />Persons reporting such occurrences by telephone shall also file a written report in letter <br />form within 15 days following first knowledge of the occurrence. This report must <br />outline the actions taken or proposed to be taken to ensure that the problem does not <br />recur. <br />IV. GROjJNDWATER REQ1111REMENTS <br />Any groundwater quality monitoring as deemed necessary by the Division of <br />Environmental Management shall be provided. <br />2. It was required in the originally issued permit that prior to beginning wastewater <br />disposal operations, two (2) monitor wells, one (1) upgradient and one (1) <br />downgradient, must be installed to monitor groundwater quality. The location and <br />construction details for these wells must be approved by the Raleigh Regional Office, <br />from which a well construction permit must be obtained. <br />3. The monitor wells must be sampled initially after construction (and prior to waste <br />disposal operations) and thereafter every March, July and November for the following <br />parameters: <br />Nitrate pH <br />Total Ammonia Total Organic Carbon <br />Total Dissolved Solids Chloride <br />Water Levels Total Organic Halides (in November only) <br />Total Coliforms <br />The measurement of water level must be made prior to sampling for the remaining <br />parameters. <br />The analytical methods used for Total Organic Halides must be capable of detecting the <br />total of all halogenated organic compounds present at a concentration of 5.0 parts per <br />billion (ppb) or greater. Any individual halogenated organic compound present at a <br />concentration equal to or above the method detection limit (MDL) must be identified, <br />quantified, and reported with sampling results. <br />The results of the sampling and analysis shall be sent to the N.C. Division of <br />Environmental Management on Form GW -59 ( Compliance Monitoring Report Form ) . <br />V. INSPECTIONS <br />Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to <br />insure proper operation of the subject facilities. <br />2. The Permittee or his designee shall inspect the wastewater treatment and disposal <br />facilities to prevent malfunctions and deterioration, operator errors and discharges which <br />may cause or lead to the release of wastes to the environment, a threat to human health, <br />or a nuisance.
