in the event the reimbursement rates ax <br />decreased, COG may, at its option, increase th <br />total number of units of service to be delivered. <br />The Agency is limited to Ywo budget adjustment <br />per funding source between January 1 and May 30 <br />Additional adjustments must be required o <br />approved by the Concerned Funding Agency. <br />f) Availability of USDA Resources. <br />Should the COG receive USDA funds for disbursemen <br />to the Agency, payments shall be made monthl, <br />based on the number of meals served by the Agency <br />which meet service standard requirement <br />multiplied by the per meal reimbursement rate se <br />by USDA. The USDA revenue must be fully accounr.e <br />for by the Agency and used during the contrac <br />period to offset the costs of services provided. <br />o. SlidincT Fee Scale for State Funded Respite Care Services <br />Caregivers receiving respite services through Socia <br />Services Block Grant funding or state appropriated caregive <br />support funding shall pay for some of the services on <br />sliding fee scale depending on their ability to pay. Th <br />income of the receipient is not considered income and th <br />income of the caregiver will not be verified. The Agenc <br />agrees to utilize the Caregiver Income Declaration For <br />(DOA -401) in determining caregiver income. This form wil <br />be completed for each caregiver during the contracts <br />renewal. Revenues generated through the sliding fee scat <br />are considered program income. <br />7. . Payments by Service Reci ients. No individual receivin <br />services under the terms of this Agreement shall be require <br />to pay any part of the cost of the service. However <br />recipients shall be given the opportunity by the agency t <br />make some contribution for the service in a manner approve <br />by COG. <br />Agency shall post a notice in a conspicuous place, statin <br />that no charge is made for the service, but tha <br />contributions will be accepted. Agency shall offer a locke, <br />box in a .convenient location for confidential deposit o <br />envelopes. All envelopes shall be collected by the Projec <br />Manager or a person designated by the Project Manager <br />Envelopes shall be opened in the presence of another person <br />the money shall be counted and the total recorded. Th <br />money shall be deposited in accordance with the usua <br />procedures of the .Agency, with a separate receipt <br />Accumulation of contributions should not exceed fift <br />dollars ($50) between deposits. <br />$. Pro ram Income. Program income is any money received b <br />Agency as a direct result of contract activity. Agenc <br />shall maintain daily records of contributions by (or o <br />behalf of) service recipients, interest received on CO <br />advance before disbursement, and any other program income <br />
