6161 <br />STATE OF NORTH CAROLINA <br />COUNTY OF ORANGE <br />AGREEMENT BETWEEN <br />WAKE COUNTY OPPORTUNITIES, INC. <br />AND <br />THE ORANGE COUNTY HEALTH DEPARTMENT <br />THIS AGREEMENT is made and entered into this the first day of June, 1990 by <br />and between Wake County Opportunities, Inc, of Raleigh, N.C., for its Head Start <br />Start Program referred to as "Head Start "; and the Orange County Health <br />Department of Hillsborough, N.C., for its Dental Clinic hereinafter referred to <br />as "Dental Clinic." <br />WITNESSETH: <br />WHEREAS, Head Start desires the services of the Dental Clinic to provide <br />dental services to the children who are enrolled at the Cedar Grove Center; <br />WHEREAS, The Dental Clinic wishes to provide dental services for Head Start; <br />NOW, THEREFORE, in consideration of the premises and the following mutual <br />promises, covenants, and conditions, Head Start and the Dental Clinic agree as <br />follows: <br />1. The Dental Clinic will provide dental examination, assessment, <br />treatment, and educational services for approximately sixty (60) children, 3 -5 <br />years old, enrolled at the Cedar Grove Center. <br />2. The Dental Clinic will assure that its employees and contract officers <br />are covered with professional liability insurance while rendering services to the <br />Head Start children. <br />
