(For SSBG add: and who are <br />not eligible for Respite Care through the <br />North Carolina Medicaid Program.) <br />Uh'?T OF SERVICE: One (1 ) hour. <br />SERVICE OBJECTIVE: To provide relief time for unpaid <br />caregivers who are caring for patients <br />who require constant supervision and who <br />cannot be left alone either because of <br />memory impairment or other problems that <br />make thew subject to wandering or make <br />them dangerous to themselves or others, <br />or because of physical immobility <br />regardless of etiology, that renders them <br />unsafe alone. Atte.cance and companion services for <br />the patient in order to provide <br />release time to the caregiver. <br />2. Personal care services, including <br />meal preparation, to the patient of <br />the caregiver. <br />Patient assessment and care planning <br />for the patient of the caregiver. <br />�.
