Revised 9/15/ HC Division of Acing Service Standareds RESPITE CARE A71ACHvl= A ERVICE DEFINITION: A service which provides needed relief to caregivers of patients who cannot be left alone because of mental or physical problems. (For SSBG add: and who are not eligible for Respite Care through the North Carolina Medicaid Program.) UNIT OF SERVICE: One (1) hour. SERVICE OBJECTIVE: To provide relief time for unpaid caregivers who are caring for patients Who require constant supervision and Who cannot be left alone either because of memory impairment or other problems that make them subject to wandering or make them dangerous to themselves or others, or because of physical immobility regardless of etiology, that renders them unsafe alone. These services are aimed toward preventing premature long —term institutionalization of the patient. PREFERRED TARGET SSBG State Caregiver Su sort Funds GROUF: Title III D and State III D Su lenient Funds: Caregivers of mentally or physically impaired adults (sixty years of age or older and /or their spouses) at risk-of instituionalization. Title III R: May also include caregivers 60+ caring for patients under 60 years of age requiring constant supervision due to mental or.physical impairment and at risk of institUtionalization. SERVICE ACTIVITIES Activities may include: 1. Atte- ndance and companion services for the patient in order to provide release time to the caregiver. 2. Personal care services, including meal preparation, to the patient of the caregiver. 3. Patient assessment and care planning for the patient of the caregiver. 4. Counseling and training in the caregiving role;. including coping mechanisms and behavior modification techniques.
