Revised 9/15/ <br />HC Division of Acing Service Standareds <br />RESPITE CARE A71ACHvl= A <br />ERVICE DEFINITION: A service which provides needed relief to <br />caregivers of patients who cannot be left <br />alone because of mental or physical <br />problems. (For SSBG add: and who are <br />not eligible for Respite Care through the <br />North Carolina Medicaid Program.) <br />UNIT OF SERVICE: One (1) hour. <br />SERVICE OBJECTIVE: To provide relief time for unpaid <br />caregivers who are caring for patients <br />Who require constant supervision and Who <br />cannot be left alone either because of <br />memory impairment or other problems that <br />make them subject to wandering or make <br />them dangerous to themselves or others, <br />or because of physical immobility <br />regardless of etiology, that renders them <br />unsafe alone. These services are aimed <br />toward preventing premature long —term <br />institutionalization of the patient. <br />PREFERRED TARGET SSBG State Caregiver Su sort Funds <br />GROUF: Title III D and State III D Su lenient <br />Funds: <br />Caregivers of mentally or physically <br />impaired adults (sixty years of age or <br />older and /or their spouses) at risk-of <br />instituionalization. <br />Title III R: May also include caregivers <br />60+ caring for patients under 60 years of <br />age requiring constant supervision <br />due to mental or.physical impairment and <br />at risk of institUtionalization. <br />SERVICE ACTIVITIES Activities may include: <br />1. Atte- ndance and companion services for <br />the patient in order to provide <br />release time to the caregiver. <br />2. Personal care services, including <br />meal preparation, to the patient of <br />the caregiver. <br />3. Patient assessment and care planning <br />for the patient of the caregiver. <br />4. Counseling and training in the <br />caregiving role;. including coping <br />mechanisms and behavior modification <br />techniques. <br />
