ATTACHMENT C <br />INTER - AGENCY LONG 'PERM CARE MANAGEMENT PROCEDURES <br />The Orange County Department on Aging provides Long Term Care <br />Management to ail county residents who are sixty years of age <br />or older. It is the process in which the indiv <br />ry idual is linked <br />to an array of seY'. -Lees appropriate to the assessed needs and <br />ased on a plan of :axe specific to the individual. Long Term <br />Care Management provides ongoing support and follow -up to <br />assure an awareness of" changes in status for a prolong <br />period, provide smoo.h coordination of services and avoid <br />unnecessary duplication. <br />Care Management differs from case management in that it is <br />Comprehens,ve and -�ng term <br />service or acencY spe • Wzereas, case management is <br />cific. <br />�n o'aer to more et ective-_v coordinate client services and <br />ensure continued appropriateness of services, the following <br />inter - agency care management procedures are instituted: <br />Upcata -he referral source -DOA Care Management Team- <br />periodical-"Y on the situation of the requested service <br />from DOA. <br />Do not refer to another service area within the Provider <br />agency without conferring with the referral source -DOA <br />Care Management Team. <br />3. tgoti;y the DOA Care Management Term promptly when a <br />specific Provider service is terminated, giving the date, <br />if possible. <br />The DOA Care Management Team should always be notified <br />when there is a problem or change in providing the agency <br />service to the client: whether on the part of the agency <br />or the client. <br />S. Purchased services shouid never be terminated or altered <br />w;thcut appropriate discussion with the DOA Care <br />Management Team. <br />PN . <br />
