(Far SSBG add: <br />not eligible for Rea and who are <br />North Carolina Medicaid Program.) <br />the <br />UNIT OF SERVICE • <br />One (1) hour. <br />SERVICE OBJECTIVE: <br />To Provide relief time for unpaid <br />caregivers who are caring for patients <br />who require constant supe )-vision and who <br />cannot be left alone either because of <br />memory impairment or other problems that <br />make them subject to wandering or make <br />them dangerous to themselves or others, <br />or because of physical immobility <br />regardless of etiology, that renders them <br />unsafe alone. These services are aimed <br />toward preventing premature lon <br />- institutionalization of the B`term <br />PrERRED TARGET patient. <br />Gam: SSBG State Care fiver Su Dort Funds <br />Title IIi D and State III D Su <br />Funds: lenient <br />Caregivers of mentally or physically <br />impaired adults (sixty <br />older and /or their Spouses) satfrisk of <br />instituionalization. - <br />Title III B: May also include caregivers <br />60+ caring for patients under 60 <br />age requiring constant supervision <br />Years of <br />due to mental or pe•vision <br />at risk of physical impairment and <br />institutionalization. <br />SB'RVICE ACTIVITIES Activities may include: <br />1. Attendance and companion services for <br />the patient in order to <br />release time to the cto Provide <br />2. Personal care services, giver. <br />meal preparation including <br />the caregiver. ' to the Patient of <br />3. Patient assessment and care plan <br />for the patien- of the caregiverning <br />y• Counseling and training in the <br />caregiving role <br />mechanisms and behavior imodification <br />techniques. <br />
