ORANGE COUNTY C.A.T. DRIVER AGREEMENT <br />This agreement entered into this 1st day of July, 1990 <br />between the County of Orange on behalf of the Department on <br />Aging, hereinafter referred to as the "County" and Orange - <br />Person- Chatham Mental Health Center, hereinafter referred to <br />as "OPC -MHC ". <br />SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY <br />I. To provide a certified driver for (1) OPC -MHC courier <br />service between sites and (2) transportation of.OPC -MHC's <br />Adult Day Treatment clients. Daily courier service sched- <br />ule attached. Daily pick -up and take -home of Adult Day <br />Treatment clients between the hours of 8 -9:30 and 3:30- <br />5:00 P.M. <br />2. To provide backup drivers in case the regular driver <br />is not available. <br />3. To follow the OPC -MHC's transportation policies and <br />procedures as well as State standards for program opera- <br />tion. <br />4. C.A.T. Transportation Coordinator will supervise dri- <br />ver. <br />SPECIFIC RESPONSIBILITIES OF OPC MENTAL HEALTH CENTER <br />1. To reimburse Orange County at $8.00 per hour for a 35- <br />hour week within 30 days after billing. <br />2. To pay for specialized driver training in the areas of <br />client behavior interaction techniques with clients /fami- <br />lies, medical management including seizures, route design <br />and any other procedures specific to the Adult Day Treat- <br />ment program. <br />3. To provide a job description specifying job duties of <br />courier /driver and a copy of required transportation poli- <br />cies and procedures. <br />4. Director of OPC --MHC Adult Day Treatment will provide <br />instructions regarding clients and their needs. <br />5. Facility Manager at OPC -MHC Northside office will pro- - <br />v de daily instruction concerning duties and coordinate <br />hange in routes with.C.A.T. Transportation Coordinator. <br />
