Form or Marti Pryor -Cook <br />TELEPHONE NUMBER - <br />Hillsborough - 732 -8181 <br />Chapel Hill - 968 -4501 <br />Mebane - 227 -2031 <br />Durham - 688 -7331 <br />PURPOSE: <br />To request approval to renew provider agreements for in -home <br />respite rare at $9.00 per hour with three identified community <br />agencies: the Home Health Agency of Chapel Hill, The Right Staff, <br />and Personalized Patient Home Assistance and other qualified <br />in -home providers that might be needed at the same hourly rate. <br />In addition, approval is requested for two community -based <br />respite rare (day rare) agreements with Charles House Association <br />(at $30 per 8 hour day) and Sharon's Way Adult Day Care (at $22 <br />per 8 hour day) and other qualified community -based providers <br />at a maximum rate of $30 per 8 hour day. (see attached agreement <br />form) <br />BACKGROUND: Triangle J Council of Governments will provide a total of <br />9 17,757 to Orange County under contract for respite care <br />beginning July 1, 1990 through June 30, 1991. There is a $647 <br />local match which is already budgeted by the Dept. on Aging. <br />The County Departments of Aging, Social Services and Health <br />have identified a need for short -term relief foir caregivers <br />of patients that do not have funds or limited amounts to pay <br />for assistance. These are family situations where the older <br />member can not be left alone due to a mental or physical <br />condition. In order to spend all the respite care funds <br />available and maintain a quality provider pool, several providers <br />will be utilized. The Respite Care Program will be administered <br />by the Dept. on Aging on a sliding fee, but coordinated by the <br />inter - departmental Case Management Team (Aging and Social <br />Services). <br />RECOMMENDATION(S): Approve agreements and authorize the Chair to sign. <br />
