Ch <br />There is no local match req <br />form) <br />BACKGROUND: The County Departments of Aging and Social Services -have <br />identified a need for the performance of critical tasks in order <br />to maintain self sufficiency within the home - such as shopping, <br />cleaning and minor repairs. In ordero spend <br />preparing meals, and main <br />all the Chore funds available maintain a quality provider <br />pool, several providers will be utilizd. The Elderly Chore <br />Service will be administered by the Dept. on Aging, but <br />coordinated by the inter - departmental Case Management Team (Aging <br />and social Services). <br />RECOMMENDATION(S): Approve provider Agreements and authorize the <br />Chair to sign. <br />
