ORANGE COUNTY C.A.T. PROGRAM 6 VEHICLE LEASE AGREEMENT FN: CATOYC- CT.DOC This agreement entered into this 1st: day of July, 1990 by and between the COUNTY OF ORANGE hereinafter referred to as the COUNTY and ORANGE PERSON CHATHAM MENTAL HEALTH CENTER hereinafter referred to as the Mental Health Center. The purpose of this agreement is to outline the conditions under which the COUNTY OF ORANGE and The Mental Health Center will cooperatively provide human service transportation to Orange County's elderly, infirmed, and disadvantaged as a participating member of the C.A.T. (Coordinated Agency Transportation) system. SPECIFIC RESPONSIBILITIES OF THE COUNTY OF ORANGE I. To lease the following identified vehicles o.r similar size at the hours specified: 1989 Dodge Van (14 pass)-, CAT #21, 7 -9am, 2:30- 4:30pm 1984 Ford Van (15 pass)- CAT# 9, 8 -10am, 3:00- 5 :00pm 2. To provide all insurance, fuel, maintenance repairs and replacement for each leased vehicle. SPECIFIC RESPONSIBILITIES OF ORANGE PERSON CHATHAM MENTAL HEALTH I. To reimburse Orange County at the per mile rate of $.35 for a van due 30 days after billing. 2• To operate and schedule the use of the leased vehicle(s) on be- half of human service clients and approved groups, according to the Orange County Transportation Board policy and procedures. 3. To park all leased vehicles at the CAT parking lot (300 West Tryon St., Hillsborough) when not in use except CAT# 21 to be parked at the Efland Cheek Elementary School in Efland. 4. To complete all required vehicle operation reports and submit to the Orange County C.A.T. Supervisor in the Department on Aging.. 5. To establish criteria for client rider eligibility. The criteria is as follows: (1) Availability of seats, (2) Inability of client to provide own transportation, (3) Where client lives and his ability to get to pickup point, and (4) Behavior on vehicle. EMERGENCY PROCEDURES. The agreed upon emergency procedures will be as follows: Two --way radios will be installed in each vehicle with the base located at the Department on Aging. In the event of a mechanical emergency, the driver will radio to base for help. In case of a medical emergency, the driver will first radio to 911 for aid and then to the base. Base operator will be responsible for contacting The Mental Health Center. At no time will the driver leave riders unattended. The term of this contract shall be from July 1, 1990 through June 30, 1991, unless sooner terminated by either party giving 30 days prior written notice to the other party of intention to terminate. IN WITNESS WHEREOF, COUNTY OF ORANGE and ORANGE PERSON CHATHAM MENTAL HEALTH CENTER have executed the Agreement of the date first above written.
