To approve and authorize the Chair to sign agency participation <br />agreements effective July 1, 1990 through June 30, 1991 with the following <br />agencies: JOCCA, OCCHS, Orange enterprises, Inc., OPC Mental Health Center, <br />Wake Opportunities - Headstart, Cedar Grove Day Care Center, Department of <br />Social Services, Health Department and Department on Aging. <br />S. APPROVAL OF PROVIDER AGREEMENT RENEWALS FOR ELDERLY CHORE SERVICE <br />To approve and authorize the Chair to sign the provider agreement <br />for Elderly Chore Service at $8.00 per hour with two identified community <br />agencies - The Right Staff and Personalized Patient Home Assistance and other <br />qualified Chore providers that might be needed at the same hourly rate. <br />Triangle J Council of Governments will provide $8,285 to Orange County from a <br />state In -Home Services Chore fund. There is no local match requirement. <br />T. APPROVAL OF PROVIDER AGREEMENT RENEWALS FOR ELDERLY RESPITE CARE <br />PROGRAM <br />To approve and authorize the Chair to sign the provider agreement <br />for in --home respite care at $9.00 per hour with three identified community <br />agencies: the Home Health Agency of Chapel Hill, the Right Staff and <br />Personalized Patient Home Assistance and other qualified in -home providers <br />that might be needed at the same hourly rate, and two community -based respite <br />care agreements with Charles House Association at $30 per 8 hour day and <br />Sharon's Way Adult Day Care at $22 per 8 hour day and other qualified <br />community -based providers at a maximum rate of $30 per 8 hour day. TJCOG wil <br />provide a total of $17,757 to Orange County under contract for respite care <br />beginning July 1, 1990 through June 30, 1991. There is a $647 local match. <br />U. AGREEMENT BETWEEN THE WAKE COUNTY OPPORTUNITIES INCORPORATED AND <br />THE ORANGE COUNTY HEALTH DEPARTMENT <br />To approve and authorize the Chair to sign the agreement between <br />Wake County Opportunities, Inc. and the Orange County Health Department to <br />provide dental services to the children of the fttwm <br />Grove Head Start Center <br />operated by the Wake County Opportunities, Inc. <br />V. CONTRACT BETWEEN UNC -CH SCHOOL OF PHARMACY AND ORANGE COUNTY <br />HEALTH DEPARTMENT <br />To approve and authorize the Chair to sign the contract between <br />UNC -CH School of Pharmacy and the Orange County Health Department to provide <br />consultant pharmacy services as required by the State to assure that <br />dispensing pharmacy functions are according to State law and regulations. <br />W. PETITION FOR ADDITION - DEERFIELD ROAD <br />To approve a petition from NCDOT for approval of the addition of <br />Deerfield Road in Little River Farms Subdivision to the State maintained <br />Secondary Road System. <br />X. PETITION FOR ADDITION - TALLYHO TRAIL AND BUGLE COURT <br />To approve a petition from NCDOT for approval of the addition of <br />Tallyho Trail and Bugle Court in Fox Meadow Subdivision to the State - <br />maintained Secondary Road System. <br />
