GOAL <br />Page 3 of 5 <br />FY 1990 -1991 <br />91- 5505 -068 <br />Contract Number <br />Health Promotion <br />Activity <br />To reduce disability and premature death due to cardiovascular disease, cancer, <br />and injury through early detection, risk factor identification, and risk <br />reduction through planned interventions and monitoring. <br />To foster the development of self - sustaining health behaviors by the employees <br />of Orange County and the residents of Orange County, by identifying risks of <br />disease and ultimately reducing these risks through a variety of health <br />promotion services. <br />Program Objectives /Planned Interventions: <br />1) By June 30, 1991, identify cardiovascular risk factors of at <br />least 50% of the Orange County employees through comprehensive screening <br />clinics and special "Know Your Numbers" screenings, and other efforts. <br />Interventions will include: <br />a) Provide a comprehensive screening program (16 clinic days) that <br />includes: Health Risk Appraisal, cholesterol, height, weight, blood <br />pressure, diet analysis, and body composition. This screening will be <br />delivered by an interdisciplinary team (health educator, nutritionist, <br />nurse). <br />b) Provide 5 cholesterol /blood pressure worksite screenings with nutrition <br />counseling. <br />c) Provide 5 cardiovascular fitness screenings through walking clinics and <br />fitness clinics. <br />d) Educate and raise awareness of cardiovascular risk factors through <br />quarterly health newsletter, 4 lunch bag seminars, and special events. <br />2) By June 30, 1991, enroll 40% of the target Orange County employee <br />population in special programs designed to change their cardiovascular risk <br />behaviors. Interventions will include: <br />a) 100% of employees participating in the comprehensive screening clinic <br />will return for individualized consultations that assist in planning <br />for health behavior changes. <br />b) Two (10 session) weight management /nutrition classes, and 4 lunch bag <br />seminars held at various county buildings. <br />c) Twice -- weekly fitness classes designed to increase cardiovascular <br />fitness, muscle strength, flexibility. <br />Reviewed by <br />DEHNR 3300 (Revised 2/90) <br />General Services Division ( Review 1/95) Initials Date <br />
