Memorandum of Agreement: Chapel Hill Carrboro City Schools and MH agency <br /> MEMORANDUM OF AGREEMENT <br /> BETWEEN <br /> CHAPEL HILL CARRBORO CITY SCHOOLS <br /> AND <br /> MH AGENCY <br /> FOR <br /> CO-LOCATED MENTAL HEALTH SERVICES <br /> This Memorandum of Agreement for co-located mental health services (the "MOA") is made <br /> and entered into this et day of October, 2017 (the "Effective Date"), between the Chapel Hill <br /> Carrboro City Schools ("CHCCS" or "District") and (the <br /> "Agency"). <br /> The purpose of this MOA is to accommodate the efficient and effective provision of mental <br /> health services for students and their families in the Chapel Hill Carrboro City Schools. CHCCS <br /> agrees to provide space for the Agency to provide mental health services to certain CHCCS <br /> students under the following conditions: <br /> 1. Agency Responsibilities and Obligations. <br /> a. Agency shall provide school-based mental health services to CHCCS students as <br /> described herein and in the Agency's response to the CHCCS Request for Proposals for <br /> the Provision of Co-Located School Based Mental Health Services, dated October, 2017. <br /> In the event of any conflict between the terms of this MOA and the RFP response, the <br /> terms of this MOA shall control. Services will include, but not be limited to: <br /> comprehensive clinical assessments; person-centered planning; individual, family and <br /> group outpatient therapy; and medication management for students receiving treatment <br /> from the Agency. <br /> b. Agency is also expected to collaborate with CHCCS on training opportunities for <br /> CHCCS staff working with students with behavioral and emotional needs; consultation <br /> with CHCCS staff on specific interventions for use with students in the school setting; <br /> participation in Child and Family Team meetings, IEP/FBA/BIP/MTSS and other team <br /> and parent meetings at the request of CHCCS staff; and parent communication on <br /> treatments and interventions. <br /> c. Agency agrees to provide services to CHCCS students at the assigned schools designated <br /> by the Executive Senior Director of Exceptional Children's Program and System of Care <br /> or designee. Agency agrees to provide the services in the space(s) provided by CHCCS <br /> at each school. Agency agrees to maintain the assigned office space in good condition. <br /> Schools may be added to or dropped from the program at the discretion of CHCCS. <br /> d.
