E. Program Evaluation and Client Satisfaction (Section 4) <br /> Describe how you will evaluate the service and provide monitoring of frequency and <br /> evaluation updates on a monthly basis to Chapel Hill-Carrboro Public Schools (note: <br /> CHCCS will also provide guidance on such data collection to ensure standardization and <br /> accountability and require monthly update meetings). <br /> • Describe how you will assess your agency's impact and success within Chapel Hill- <br /> Carrboro Public Schools. <br /> • Explain how the agency will ensure ongoing internal monitoring and quality <br /> assurance within the outpatient clinic programs and services (i.e. pre and post <br /> parent/teacher satisfaction surveys). Also describe how you plan to use the North <br /> Carolina Treatment Outcomes and Program Performance System (NC TOPPS) or <br /> your agency's tool to determine level of treatment fidelity and outcome. <br /> F. Corporate Structure and Financial Condition (Section 5) <br /> Section 5 shall include a description of the contractor's corporate form, the date the <br /> contractor was incorporated, and the names and official titles of all its corporate officers. <br /> In addition, Section 5 of the proposal shall include authoritative documentation of the <br /> contractor's financial condition and stability. Such documentation shall include an audited <br /> 2015 financial report and any additional documentation on the contractor's financial <br /> condition and stability that the contractor wishes CHCCS to consider (e.g. evidence of <br /> general liability, workers compensation, and medical malpractice insurance). <br /> Also, describe your administrative supports to manage the services (e.g. bookkeeping, <br /> accounting, insurance billing, management of confidential medical records, and other <br /> clerical supports). <br /> G. Experience, Background& References (Section 6) <br /> Section 6 of the proposal shall include a detailed summary of the contractor's experience <br /> and background in providing treatment and care for adult, child and adolescents. <br /> Section 6 of the proposal shall also include a list of at least three (3) but no more than five <br /> (5) references for individuals with firsthand knowledge of the contractor's background, <br /> experience, qualifications, and reliability, including the names and contact information for <br /> each such reference, the length of time each reference has had direct knowledge of and <br /> experience with the contractor, and the relationship of the contractor to each reference. <br /> 122 <br />
