Older adults are connected to and supported to live in housing options of their choice. <br /> RESPONSIBLE AGENCIES: OCDOA, with support from Orange County and Town of Chapel Hill Housing <br /> Departments,TJCOG <br /> C, y m ,Collaborate across repair/remodel organizations to better communicate, share <br /> cases, and refer to specialized services. <br /> INDICATORS <br /> 3.6.2a. Network of repair/remodel organizations is developed. <br /> 3.6.2b. Collaboration coordinator is selected. <br /> 3.6.2c. Representative from each organization is designated to network. <br /> 3.6.2d. Referrals are increased across organizations. <br /> 3.6.2e. Multiple repairs are provided by multiple organizations through use of coordinated repair <br /> network. <br /> 3.6.2f. More comprehensive repairs are provided to residents. <br /> 3.6.2g. Data are collected and shared regarding safety and well-being of residents who receive home <br /> repairs or modifications. <br /> 3.6.2h. Funding is increased for repairs and remodels that partially, but not entirely, bring a home up to <br /> code. <br /> 3.6.2i.Training is developed for OCDOA employees and others who make home visits regarding home <br /> safety resources and services. <br /> RESPONSIBLE AGENCIES: OCDOA, Orange County Housing Department, Orange County Planning <br /> Department, Orange County Health Department, Seniors and Law Enforcement Together(SALT), Habitat <br /> for Humanity, Orange County Fire Marshal <br /> 27 <br />
