Certificate of Disposal must be received by County <br /> at least every six (6) months. <br /> IV. GENERAL REQUIREMENTS FOR SELECTED PROVIDER <br /> A. Details of Services <br /> 1. Provide collection service six (6) days per week at the Orange County Landfill location, 1514 <br /> Eubanks Road, Chapel Hill, NC 27516 <br /> 2. Provide collection up to six (6) days per week at the Walnut Grove SWCC, 3700 Walnut Grove <br /> Church Road, Hurdle Mills, NC 27278. <br /> 3. Adjust the level of service as needed to meet budgetary constraints, address management <br /> requests, etc. <br /> 4. Reporting <br /> Beyond those requirements listed In Section III, Subsection F, Provider shall submit monthly <br /> and annual reports to the County. The required monthly reports shall be submitted with invoice <br /> for services, and shall include <br /> • A breakdown of material accepted by weight by Department of Transportation (DOT) <br /> Hazard Class, including the disposal method used to manage this material. This last <br /> monthly report must be useful in order to allow County to complete the required State <br /> of North Carolina Hazardous Waste Facility Annual Report. <br /> • The following operational records required by state of North Carolina shall be supplied <br /> by the Provider in a format as requested or required by state (i.e. total pounds <br /> collected by type, and the treatment used for each type): <br /> • Compressed gases <br /> • Flammable solids <br /> • Oxidizing material <br /> • Poisonous material <br /> • Radioactive material. <br /> • Corrosive material <br /> • Freon Cylinders <br /> • Hg and Hg Debris <br /> • PCB Ballasts <br /> • Latex Paint <br /> • Regulated material <br /> • Non-Regulated material <br /> • Other <br /> 5. The following operating records (data) are required by Orange County and shall be provided by the <br /> Provider. Invoices shall also be submitted in an electronic format. Data shall include, but not limited <br /> to <br /> - Customers served (total each day) <br /> - Origin of customer (Orange, Chatham, Durham, Wake) (only applicable at Eubanks <br /> Road location <br /> - CESQG served (only applicable at Eubanks Road location) <br /> - CESQG Pounds (only applicable at Eubanks Road location) <br /> - Materials collected <br /> - Material pounds/units/quantities <br /> Page 9 <br /> HHW RFP#5183 <br />
