OWNER / MAINTENANCE AGREEMENT <br /> 1. Grantee shall retain ownership of the materials and equipment purchased <br /> with grant funds for a minimum of five years. Grantee may request to sale <br /> or dispose of grant funded equipment by written notification to the Agency <br /> and with written Agency approval. The Agency shall have 30 days to <br /> respond to the grantee. Any proceeds over$100.00 generated from the sale <br /> or disposition of equipment purchased under this grant shall be returned to <br /> the Agency within 30 days of sale. <br /> 2. All equipment purchases using grant funding shall be identified by the serial <br /> number and picture identification by the Grantee and provided to the Agency <br /> upon purchase and reimbursement request. <br /> 3. The Grantee shall keep all equipment in working order and maintained based <br /> on manufacturer's recommendations. <br /> 4. For the first five years of ownership: <br /> a. Grantee shall maintain and keep on file a record of all repairs for <br /> inspection purposes. At a minimum this re_ cord shall include the <br /> following information: <br /> ❑ Date of repair; <br /> ❑ Reason for repair; <br /> ❑ Cost of repair; <br /> ❑ List of replacement parts if any; <br /> ❑ Name of company and person who completed repair work; and <br /> ❑ If a warranty is associated with any replacement parts, it should <br /> be placed on file. <br /> b. Grantee shall keep manufacturer's warranties on file. <br /> c. Grantee shall follow the "Agricultural Development Project Monitoring <br /> Policy and Guidelines" and amendments there to. <br /> 5. Any changes to this agreement shall require written approval by the Agency. <br />
