The rating <br /> should not be used by the <br /> supervisor to avoid the <br /> responsibility of informing an <br /> employee of unsatisfactory <br /> performance. <br /> 3.3 Performance evaluations will be prepared by the immediate supervisor <br /> of each employee and reviewed by the appropriate department head. <br /> An employee in a supervisory position who is leaving the position <br /> will be required to submit performance evaluation forms on all <br /> employees under their supervision who have not been evaluated within <br /> the previous six month period. <br /> 3.4 The supervisor should hold an annual performance evaluation <br /> conference with each employee. If an employee disagrees with any <br /> statement in an evaluation, the employee should submit statements or <br /> comments of disagreement. Both the employee and supervisor must <br /> sign the performance evaluation form. <br /> 3.5 Performance evaluation forms are confidential and will be <br /> made available only to: the employee evaluated or the <br /> employee's assigned representative; supervisor; department <br /> head; Personnel Director; County Manager; County Attorney or The <br /> Governing Boards. <br />
