Monthly Expense and Project Evaluation Reports -After the Effective Date of the <br />Agreement, these Reports are due on or before the twentieth (20°i) day of each month for the <br />prior month. <br />c, Six (6) Month Progress Reports -After the Effective Date ofthe Agreement, these reports are <br />due as specified by Commission staff at six (6) month intervals. <br />d, Consolidated Annual Report -Due as specified by Commission staff.. <br />e. Annual Certification of Account Balance -Due annually on or before December 31 during <br />the Agreement Term. See Attachment E-2 for format. <br />f, Final Project Report-Due within sixty (60) days after end of last Grant Cycle including any <br />Contract Performance Period Extension. At a minimum this Report shall demonstrate how <br />successfully the Project met its goals and performance obligations. <br />g. Reports for Outcome Evaluator(s) -Due on dates specified and in the format specified by <br />OE's and approved by the Commission. Grantee agrees to cooperate with OE's who wilt <br />conduct periodic site visits and telephone interviews. <br />h Program Specific Reports -Those Reports set forth on Attachment G that are unique to the <br />Program this Project supports. <br />i. Additional Reports -Additional information, which will assist the Commission with <br />evaluation of the short and-long range impact of its Programs and this Project. The Grantee <br />recognizes that such requests may occur during or after termination of this Agreement. <br />Grantee agrees to use its best efforts to provide any and all Reports requested by the <br />Commission in a timely and professional manner and in the format requested by the <br />Commission. <br />j Legally Required Reports- As required by North Carolina General Statutes: Complete details <br />are available at www.ncauditor.net. <br />(I) Non-Governmental Entities -Non-Governmental Entities are required to comply with <br />N.C. General Statute § 143-6.2 in the format required by the State Auditor. Grantee <br />agrees to submit all reports required in compliance with N.C. General Statute § 143-6.2 <br />via electronic mail, except for any Certifications requiring asignature/notary which will <br />be sent via postal mail. <br />(2) Governmental Entities -Governmental entities must comply with the state's requirement <br />for annual independent audits pursuant to N.C, General Statute § 159-34. <br />C. Project Records and Visits <br />The Grantee agrees to the following: <br />1. Record Retention <br />a. Five (5) Years Following Termination Date - For a period of five (5) years following the <br />Termination Date, Grantee shall maintain full, accurate and verifiable financial records, <br />supporting documentation, and all other pertinent data for this Grant and this Project, These <br />records must be maintained so as to identify and document cleazly the expenditure of <br />Commission funds provided under this Agreement separate from accounts for other awazds, <br />10 <br />
