The format and <br />requirements of this contract are the same as for other such sterilization service contracts that <br />have previously existed and/or are being recommended for renewal for the coming fiscal year. <br />More specifically, the contract fee for sterilization is the flat rate of $60 per animal; and for each <br />sterilized animal, a physical examination shall be conducted and records prepared for Orange <br />County's Animal Shelter. Also, there are select additional predefined charges for certain <br />services that can be performed on sterilized animals but only as authorized by management of <br />the County's Animal Shelter. These are enumerated in Appendix A of the attached renewal <br />contract. <br />The sterilization surgeries authorized by this contract are to be performed by Dr. James Miller, <br />DVM, at the new North Churton Street Animal Hospital in Hillsborough. Dr. Miller has practiced <br />at the Triangle Emergency Animal Hospital and currently owns two veterinary practices. On <br />May 14, 2006, he submitted the only proposal that met the County's service needs in response <br />to a request for proposals far such sterilization services. Dr. Miller did so in response to a letter <br />to area veterinarians that was developed by asub-committee of the County management team, <br />consisting of Bob Marotto (Animal Services Director), Pam .Jones (Purchasing/Central Services <br />Director), and Jae Pulcinella (Shelter Director). <br />Bob Marotto (Animal Services Director) and Joe Pulcinella (Animal Shelter Director) conducted <br />
