POLICY #: 4,15 PAGE 4 OF 7 <br />Duties and responsibilities of the afternoon shift Med Passer: ~/ <br />Clinic Preparation contd; ~! <br />• Med passer will check each Inmate's Chart to ascertain if any medical records are needed from <br />another Jail, Hospital, or other Medical Provider for the Jail Health Provider. <br />• If past medical reconis need to be obtained for review by Jail Health Provider or if they have been <br />requested to be obtained by Jail Health Provider, a recorcis release should be signed by the inmate, <br />The Medical Facility or other DetentioNCorrectional Facility care was obtained should be contacted <br />by the Med Passer and records release should be faxed.. If information requested is needed for <br />same day clinic, the urgency of obtaining infonnaticn should be conveyed to the prior Facility to <br />expedite receipt of information. Documentation of contacts made to other Facilities is imperative. <br />Names of those contacted, phone number and fax numbers should be documented on a Progress <br />note and placed in Inmate chart. If receipt of information is expected during shift other than that of <br />the Med Passer who requested the information, this should be verbally conveyed to the oncoming <br />Med Passer/Shift Supervisor. <br />Alert the on Duty Supervisor of the Clinic schedule to prepare staff of Inmate transport to clinic. <br />11. Assistance of Jail Health Provider in clinic. <br />• Any Inmate Medical RequesVlntake form the Med Passer feels is priority should be verbally <br />communicated to the Jail Health Provider upon arrival to the Clinic. <br />• The Jail Health Provider and Med Passer will compile the list of Inmates to be seen. The Med <br />Passer will give the list of Inmates to be seen in the Clinic to the Detention Officer designated by the <br />Shift Supervisor to transporUsupervise Inmates to the Clinic. <br />• The Med passer will assist the Jail Health Provider as needed during clinic. <br />Med passers will write notes on any special orders the Jail Health Provider may give for an Inmate, <br />• Med passer assisting in Clinic is responsible for execrrtionlcommunication of any orders written <br />during Clinic. <br />• Med passer will communicate with Shift Supervisor/Lieutenant of any behavior concems/suicide <br />watches recommended by.lall Health Provider during clinic, <br />• Inmate charts wtth medication orders that have been completed should be placed in Medication <br />Completed box. Med Passer receiving new medications from Pharmacy will have these charts to <br />compare new medication orders. <br />Medications to 6e initiated as NOW/STAY orders should be transcribed per Jail Protocol on Inmate <br />MAR. <br />• Medication refill forms completed during Ginic shall be faxed to Pharmacy facility, These forms <br />should be placed In Medication Completed box for review of Med Passer receiving medications <br />from Pharmacy facility, <br />Treatments should be documented on Inmate MAR per Jail Protocol. Verbal communication to next <br />Med Passer of any unusual treatments/circumstances or information Jail Health Provider wants <br />obtained later in shift should be discussed during shift report. <br />12. Federal Inmates Responsibilities <br />• Upon arrival of new Federal Inmates, the Med Passer should be available to review Medical History, <br />medication information, Intake form and any new Medical Requests. <br />• Review medicationslmedical records, if information is needed from transfening facility, that facility <br />should be wntacted immediately and information obtained verbally or faxed to our facilfty, <br />• Jail Health Provider should be contacted in reference to information obtained on new Federal <br />Inmates. Orders given should be documented in the Inmate chart on a Progress sheet and on MAR <br />as appropriate. <br />• Obiain list of Inmates leaving the facility from the on Duty Supervisor. <br />Preparation of Federal Inmates for departure is to be done by the Med Passer the day before <br />departure.. <br />• Medical records are to be copied of any Federal Inmate leaving the facility, <br />• Medications are to be packaged for transport with inmate, <br />
