POLICY #: 4.15 PAGE 4 OF 8 <br />Duties and responsibilities of the morning daytime shift Med Passer: x.48 <br />9. contd; <br />Medication orders obtained to be initiated as STAT or Next Med Pass, shall be <br />documented on the Inmate's MAR according to Jait Protocol. <br />Medical Intake forms should be screened for medications to be initiated and significant <br />health problems (Heart disease, Diabetes, Seizures, Alcohol/Drug use daily, etc.) The <br />appropriate Provider shall be contacted at time of review of the intake for initiation of <br />medication. Any Health problem in question of significance should be discussed with a <br />Provider. Information about medications to be initiated (name, dose, frequency) should <br />be ascertained from the Inmate by the Med Passer prior to contacting the Provider. <br />However, obtaining medication information from the Inmate should not delay contacting <br />the Provider for any sign cant length of time. ff information is not readily available, the <br />Provider should be contacted with this information. <br />Med Passer conducting preliminary screening of inmate i.e.; blood pressures, vital <br />signs, urine dips, accu checks, etc. shall be written on the medical request form. When <br />completed the results of each vital sign taken of any inmate shall be written on the <br />inmates medical request fore and be returned to the Nurse Practitioner (medical <br />professional) file in the med room paper clipped to the front of existing or new inmate <br />folder. The Provider shall be contacted about above information obtained if the Med <br />Passer feels information needs to be reviewed prior to next scheduled clinic day. <br />10. Clinic Preparation <br />Clinic scheduled during the Moming Med Passer shift shall be prepared by the Moming Med <br />Passer. <br />All Inmate Medical requests and medical records shall be prepared by the Moming Med <br />Passer prior to the arrival of the Jail Health Provider. <br />• Med passer shall attach Medical Request form, Clinic Documentation Form, laboratory <br />results, and Inmate records obtained to the Inmate chart and place in Clinic box. <br />• Med passer will check each Inmate's Chart to ascertain if any medical records are needed <br />from another Jail, Hospital, or other Medical Provider for the Jail Health Provider. <br />If past medical record need to be obtained for review by Jai) Health Provider or if they <br />have been requested to be obtained by Jail Health Provider, a records release should be <br />signed by the inmate. The Medical Facility or other Detention/Comectional Facility care <br />was obtained should be contacted by the Med Passer and records release should be <br />faxed. If information requested is needed for same day clinic, the urgency of obtaining <br />information should be conveyed to the prior Facility to expedite receipt of information. <br />Documentation of contacts made to other Facilities is imperative. Names of those <br />contacted, phone number and fax numbers should be documented on a Progress note <br />and placed in Inmate chart.. If receipt of information is expected during shift other than <br />that of the Med Passer who requested the information, this should be verbally conveyed to <br />the oncoming Med Passer/Shift Supervisor, <br />Alert the on Duty Supervisor of the Clinic schedule to prepare staff of Inmate transport to <br />clinic. <br />
