(~ <br />POLICY 4.14, "MEDICAL RECORDS" <br />PAGE 3 OF :3 _ <br />3. Release of Medical Information/Records to Other Authorities: <br />A. Medical records should not be allowed outside the facility, However, in special <br />situations (e.g,, an attorney may request a medical record for a legal case involving the <br />inmate; another health care provider may request the file), it may become necessary to <br />release an inmate's medical record to an outside authority. In these cases, jail officers <br />must receive a written consent form from the inmate authorizing his/her release of their <br />medical record to the authority. Under no circumstances will any inmate medical <br />record be released to any authority without the written consent of an inmate. <br />B. Medical information for inmates being transferred to another correctional facility <br />will be handled in the following manner: <br />1, A sealed written statement from the Facility Medical Personnel may be <br />provided the transporting officer for delivery to the receiving institution's health <br />care provider, The written statement will include general information concerning <br />the inmate's medical history and will authorize the receiving facilities medical <br />staff to contact the physician for more information. <br />2. Should a sealed, written statement not be provided the transporting officer, <br />the Facility Medical Personnel will contact the receiving facilities medical staff <br />and verbally provide information concerning the inmate's medical history to <br />him/her, <br />C. The Facility Medical Personnel will be responsible for advising the Director of <br />Health Services, Division of Prisons, whenever an inmate is transferred from the facility <br />to the Department of Corrections, who is known to be infected with HIV or AIDS. <br />4. Retention of Inmate Medical Records: <br />A. Inmate medical records will be maintained for a period of at least three (3) years <br />as required by state standards. <br />B. Facility Medical Personnel will be responsible for ensuring that all inmat <br />medical records are properly maintained pursuant to state standard~s~ <br />_ Reviewed and Atrroved: <br />(Sheriff) <br />Date: i_L,~1~~~U~ <br />
