POLICY 4.14, "MEDICAL RECORDS" <br />PAGE20F3 <br />~3 <br />D. The following documents/information will be required to be filed in the inmate's <br />medical record: <br />1. Preliminary Health Screening Form and Inmate Medical Screening Report; <br />2. Medical evaluation results (usually recorded on a "Prisoners Medical Report <br />Form"}; <br />3. Medical information/prescription information; <br />4. Reports of laboratory, x-ray and any diagnostic tests; <br />5 Progress notes; <br />6. Inmate consent and refusal of treatment forms; <br />7. Release of information forms; <br />8. Hospital discharge forms; <br />9. Special treatment plans; and/or, <br />10, Medical appointment information, to include the time, place, date and with <br />whom the appointment is with. <br />2. Access to Medical Records: <br />A A locked filing cabinet will be maintained in the Medical Examination Room of <br />the facility for the purposes of filing inmate medical records. <br />B. Only the following individuals will be allowed access to medical records under the <br />following circumstances: <br />1. Unlimited access to inmate medical records will only be granted to <br />authorized medical professionals. <br />Z. Limited access will be granted to the .Jail Administrator and/or Sheriff <br />during emergency situations when either the physician or other medical personnel <br />are unavailable at the facility to access such records; <br />3. Upon the written consent of an inmate, or when the Jail Inspector as part of <br />his inspection asks to check inmate's medical records far proper filing and storage. <br />Pursuant to state law, an inmate must be given written notice of his/her right to <br />object prior to allowing a Jail Inspector access to his/her record (NOTE: Tlris <br />information is provided to inmates in the Inmate Handbook), <br />
