3~ <br />POLICY 4.09, "ADMINISTRATION AND CONTROL OF MEDICATIONS" <br />PAGE 7 OF 9 <br />a. Each inmate's medication will come from the contracted phazmacy <br />on a card. The card shall include each unit dose and labeled with uunates <br />name, medication strength, dispensing dose, route and time. Should a <br />medication be in liquid, cream or powder form, small medicine cups will be <br />used and the proper dosage poured into the cup; according to the <br />medication label on the container. <br />b. The inmate's name will be printed or labeled on the outside of each <br />cup; each inmate's medication will be placed from the card(s) into the <br />medication cup at medication administering time. PRN medications will <br />only be placed in medication cups at the inmates request for the medication.. <br />Powder, cream or liquid medications will be placed in medication cups at <br />the cell where the medication is to be administered. <br />a Once completed, all medications will be placed on a cart to be used <br />for purposes of carrying the medications to each inmate. <br />4 Once the steps listed in 3 (a) tluough (c) have been completed all medications <br />shall be returned to their proper medication cart drawer storage azea. Cards shall <br />be filed by cell assignment, last name alphabetical order, and medication order <br />according to the MAR. Extra medication cards will be stored in the bottom <br />drawer of the medication cazt in cell assignment last name alphabetical order and <br />medication order according to the MAR. At the completion of each medication <br />pass, the Medicine Administering Officer will organize, as previously listed, the <br />medicine cart and M.A.R. for the next med pass to begin. The ,jail detention <br />medical administering officer shall obtain the Medication Administration Record <br />L,og Book and begin distribution of medications to the appropriate inmates. The <br />designated officer will, to the extent possible, ensure that another officer <br />accompanies himlher when making medication rounds. <br />5. The designated officer will visit each inmate requiring medication at his/her <br />housing unit to administer medication. The officer will be required in the presence <br />of the inmate to double-check that the proper dosage is administered. After <br />administering the inmate's medication; documentation shall be recorded on the <br />MAR to confurn that medication was received or refused. <br />6 the designated officer will observe the inmate take his/her medication to <br />discourage hoarding of medications by any particular inmate. <br />7 Once an inmate has been provided his/her medication, the designated med <br />passer will record their initials in the box(es) at the date, medication and <br />administering time on the page with the following information in the Medical <br />Drug Log Book; indicating the same. <br />a. Name of inmate; <br />b. Type of medication edi <br />c. Dosage amount; <br />d. Time medication was taken; <br />
