POLICY 4.09, "ADMINISTRATION AND CONTROL OF MEDICA'T'IONS" ~5 <br />PAGE3OF9 <br />4 Non-prescription med pass procedures <br />1 All non-prescription medication will only be given during one of the four <br />regular med passes by certified med passers. <br />2. The procedures for non-prescription medication must be done in the <br />following order: <br />A. the inmate must request the non-prescription medication <br />B. The officer is to ensure that the inmate has a request form to fill out for the non- <br />prescription medication and the follow up visit with the doctor <br />C The officer is to complete an inmate sick call request form, staple it to the <br />inmate's request form, and hand it to the shift supervisor <br />D, The shift supervisor is to; <br />1. Read the medical information on the form <br />2 Initial, time and date the form <br />3. Initial and log the form in the medical forms <br />received log <br />4 Place the form in the inmate medical sick call <br />request form holder in the medical office <br />E. The following items aze to be done by the medical personnel on duty or the <br />designated med passer if medical personnel are not on duty: <br />1. Write the order for the non-prescription medication to be <br />administered on the sick call form according to the protocol <br />2. For inmates already on medication, attach the appropriate non- <br />prescription medication label or transcribe the order to the mar, and <br />complete according to the mar and protocol procedure guidelines, <br />3. For inmates not already on medication, take a blank mar and attach <br />inmate photo id label, attach the appropriate non-prescription medication <br />label or transcribe the order to the mar, ,and complete according to the mar <br />and protocol procedure guidelines. <br />4. Place any new mar in the maz log book <br />5. Take the non-prescription medication card from the locked cabinet <br />and complete the form verifying the amount of medication removed. <br />
