aq <br />POLICY 4.07, "MENTAL HEALTH CARE DELIVERY SERVICES" <br />PAGE 3 OF 3 <br />D. The contract medical professional may make any of the following decisions: <br />1. The physician may travel to the facility to examine the inmate; <br />2. The physician may request that the inmate be transported to his/her <br />office for examination, or, <br />3. The physician may recommend that our Contracted Psychiatrist see the <br />inmate. <br />4. The contracted medical professional, contracted Psychiatrist, or EMS may <br />direct you to transport the inmate to ZINC-Chapel Hill Hospital. <br />E Any recommendation requiring an inmate to be transported outside the facility for <br />treaimentlexamination shall be carried out in accordance with Orange County Jail Policy <br />4.1.3, "Transportation of Inmates for' Medical Care." <br />F. Under no circumstances shall any Orange County Jail Detention official other than <br />the contracted medical professional make any determination that any inmate is in need of <br />Mental Health Caze. jnote: when an emerEency exists and when medical staff is not <br />on duty the supervisor must contact the-Lieutenant Maior or Sheriff a decision will <br />be made. <br />G. Once a decision has been made by the physician as to the treatment of the inmate, <br />the responding detention officers will be required to complete a "Prisoner Medical Report <br />Form." The form should contain, at a muumum, the following information: <br />1. The problem(s) the inmate was having; <br />2. Any action taken by,jail detention officers; and, <br />3. Any instructions made by the physician that were carried-out by the detention <br />officers.. <br />H. All completed "Prisoner Medical Report Forms" shall be filed in the inmate's <br />medical record. Documentation regarding the incident will be recorded on the Medical <br />Log by the shift supervisor.. <br />I. .Tail detention officers will also be required to complete an official Incident Report <br />regarding the matter for submission to the .Tail A strator Sheriff by the next <br />working day. ~ , ~~ <br />Reviewed and Approved: ~~~ <br />(Sheriff) <br />Date: ?lll.~a l 2-°° y <br />
