POLICY 4.03, "MEDICAL COMPLAINTS AND REFERRALS" ~~ <br />PAGE 2 OF 3 <br />5. Documentation: When an inmate is admitted to the Orange County .Tail, as stated the <br />Detention Booking Officer shall complete a Medical Questionnaire. If the inmate on <br />admittance requests or we find in our opinion on admittance that the inmate needs to see a <br />medical professional, the booking officer shall complete a "Prisoner Medical Request <br />Report", OR if during a shift an inmate request or if we fmd an inmate in need of medical caze, <br />we in each situation shall file a "PRISONER MEDICAL REFERRAL /REQUEST <br />REPORT". <br />It shall be the duty of each day AND night detention shift supervisor on duty to collect and <br />record on the prisoner medical report log in the Medical Room all new reguests or referrals <br />for inmates, and "PRISONER MEDICAL REPORT FORMS", The log shall have the name <br />of the inmate, date and time he/she was admitted OR REQUESTED MEDICAL TREATMENT, <br />OFFICER COMPLETING THE FORM AND SUMMARY OF COMPLAINT OR FORM. The <br />medical report shall have the date and time the inmate requested (asked) for medical treatment <br />and detention officer who received the request.. The detention officer taking the medical request <br />(complaint) shall summazize the complaint on the report, The Shift Supervisor, Med Passer, and <br />Medical Professional receiving the form shall initial and mark the status of the medical request <br />form in appropriate boxes. Upon completion, the on-duty Detention Supervisor shall present the <br />forms to the on-duty med passer.. It is the duty and responsibility of the on duty med passer to <br />wive the "PRISONER MEDICAL FORMS" to the contracted facility Medical Professional <br />on duty for that day in the Orange County Detention Center, <br />It is the duty and responsibility of the med passer to have the medical professional <br />receiving the forms to initial or sign the medical log demonstratine that they have received all <br />the forms from the detention med passer.. It shall then he the duty of the contracted medical <br />professional to review each prisoner medical form, complaint and prioritize each inmate's <br />medical complaint in terms of health caze needed. NOTE: ~• not fail it is the Shift <br />Supervisor and Detention Officer's duty and responsibility to insure that collectinE and <br />~resentin2 of all recently admitted "Prisoner Medical Forms" to the med passer, whose <br />duty it is to ensure they give the forms to the medical professional as prescribed with <br />documentation.) <br />The "PRISONER MEDICAL FORMS RECEIVED LOG" shall be signed by each of the <br />following the Detention Supervisor, med passer and the contracted medical professional <br />receiving the forms. If the medical professional is not on duty to receive the PRISONER <br />MEDICAL FORMS, they shall be placed in the PRISONER MEDICAL FORM BOX in the <br />medical room in the jail,(Remember we can not fail to have the medical professional to sign <br />or initial receiving the forms) <br />
