Other demographic information. <br /> V. Performance Monitoring and Quality Assurance: <br /> The NC OMHHD and CCCPH will monitor the Local Health Department by conducting site visits and <br /> audits (minimum of one per year)and reviewing the required reports to determine if program <br /> deliverables are being met.The Local Health Department will be visited at least once during the service <br /> period of this Agreement Addendum and there will be routine correspondence between the Regional <br /> MDPP Coordinators and the DPH MDPP Program Consultants.If deliverables are not being met,the <br /> Revised June 2016 <br />
